Services: Health Links – Info Santé, is available to all Manitobans to help them figure out or get help for any illness. Registered nurses follow assessment guidelines to triage health issues – everything from flu symptoms to child fevers to heart attacks. Callers are given advice as to what health-care path should be followed: call 911; visit an emergency department, urgent care, walk-in clinic or family physician; or advice and education are given about managing symptoms at home. Health Links – Info Santé nurses also provide general health information and help callers find health resources in local communities across Manitoba and is available in over 100 languages through over-the-phone interpreters.

Services Offered: Nine Circles Community Health Centre, with expertise in the care and treatment of HIV, Hepatitis C and other sexually transmitted infections, delivers comprehensive primary care, social support, education and prevention services – creating healthier communities for Manitobans. Nine Circles offers a primary care clinic in Winnipeg for clients aged 13 or older who have HIV or are at higher risk of contracting HIV. They also offer harm reduction supplies and have a variety of healthcare resources on their website.

Services: The Portage Clinic is part of Southern Health and is a walk-in medical clinic. The clinic is open from 9am-8pm on Mondays-Thursdays, 9am-7pm on Fridays and 11am-4pm on Saturdays and Sundays. The walk-in clinic should be used for non-emergency medical needs only. Services: The Portage General Hospital provides a wide range of services, including diagnostic imaging (CT scans, ultrasound and x-rays), laboratory services, obstetrics, community cancer services and more. Telehealth services, including a mental health liaison nurse, are used to connect clients to health care providers at other locations in Manitoba, reducing the need to travel for care. Dialysis treatments are also available on site.

Services: TeleCARE Manitoba is a telephone-based program that helps Manitobans with heart failure or Type 2 diabetes manage their condition. TeleCARE Manitoba is built on the successful congestive heart failure program operating since 2005 out of the Provincial Health Contact Centre.
Eligibility: Targeted to Manitobans with limited access to chronic disease management services and supports.

Hours:  Monday-Friday: 8:00 am-8:00 pm
Additional Information: At TeleCARE Manitoba, health care providers specialized in chronic disease self-management contact patients by phone based on an individualized call monitoring schedule. During these phone calls, program staff assess the person’s health, monitor any symptoms or problems, and provide education and tools to help patients to better manage their own health. The team of health care professionals at TeleCARE Manitoba also works with the person's family physician/primary health care provider to address any health concerns. For people without a telephone, this service can be provided by video conference through their local MBTelehealth site.
